Join our team of competent and compassionate professionals as a Care Coordinator in the BHCP program at Behavioral Health Network!

Your experience, knowledge and great skills will support the BHCP program with the following:

  • Providing telephonic and in-person Care Management and Care Coordination support to members based on the collaborative spirit of the integrated health model that promotes.
    improved health, independence, and self-sufficiency.
  • Providing services to members in the community (e.g., member homes, health clinics, collateral providers, supportive services, other community settings), helping them to
    connect to needed resources and to overcome barriers impacting their care.
  • Spending approximately 50% of time supporting members in the community.
  • Providing transportation to members using personal vehicle when necessary.
  • Providing services to members out of the area and in other parts of the State when necessary.
  • Acting as primary "point of contact" for all members on caseload ensuring that members' needs are met.
  • Including all appropriate parties in the individual's care including primary care providers,
    therapists, nurses, and others as indicated by the member.
  • Facilitating collaboration and consultation with internal and external service providers to meet member needs in a timely and efficient manner.
  • Making referrals to other services and resources as needed.
  • Providing crisis management support to members helping to reduce the risk of harm to self or others.
  • Completing high quality documentation and completes within the required timeframes per program standards.
  • Documenting all member contacts in the appropriate electronic health record(s) within 24 hours of contact.

The Behavioral Health Community Partners (BHCP) team at BHN has extensive experience in managing and coordinating care for adults ages 21-64. It holds each individual at the core of what it does. Through the coordination and integration of care, BHCP assists individuals with medical, behavioral health, or substance abuse concerns in navigating the health care system and improving their health outcomes.

Behavioral Health Network is a growing non-profit community behavioral health agency that has been providing services to children, adults, families and communities in Western Massachusetts since 1938. BHN provides comprehensive, outcome-driven behavioral health care. We are dedicated to offering high quality, affordable and culturally appropriate care to people of all ages and income levels in our constituent communities.

What are we looking for in a Care Coordinator?
  • Bachelor's degree in social work, Human Services, Nursing, Psychology, Sociology,
    or related field; or
  • An associate degree and at least one-year professional experience in the field; or
  • A High School Diploma and at least three years of relevant professional experience.
  • Bilingual, Spanish and English Speaking preferred.
  • Demonstrates cultural and linguistic competence in dealing with members with a variety of different cultural backgrounds and linguistic needs.
  • Demonstrates willingness and ability to treat challenging and diverse member populations.
  • Demonstrates working knowledge of community resources and how to help members access them.
  • Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management.
  • Professional demeanor in communicating with members, colleagues, and other providers.
  • Familiarity with Internet, email systems, and electronic health records (EHRs).
  • Reliable and legally registered vehicle required.
  • Valid driver's license required.
