Join our team of compassionate and competent professional as a Care Coordinator in the One Care program at Behavioral Health Network! Care Coordinators are the individual's primary contact for questions, team decisions, and services. You are your team will empower and promote changes needed to facilitate positive health outcomes with challenging and diverse health populations. You will expand your knowledge base by completing mandatory and interest-guided optional trainings and your working knowledge of community resources.

One Care holds each individual at the core of what it does.Through the coordination and integration of care, One Care assists adults ages 21-64 with medical, behavioral health, or substance abuse concerns in navigating the health care system and improving their health outcomes.

Behavioral Health Network is a growing non-profit community behavioral health agency that has been providing services to children, adults, families and communities in Western Massachusetts since 1938. BHN provides comprehensive, outcome-driven behavioral health care. We are dedicated to offering high quality, affordable and culturally appropriate care to people of all ages and income levels in our constituent communities.

Care Coordinators at Behavioral Health Network's Care Management program have the following qualities:

  • Knowledge of mental illness, the interface between mental health and medical conditions and systems, substance abuse, and care management.
  • In lieu of a Bachelor's degree, a High School Diploma or equivalent with 2 years of work experience in a related mental or behavioral health field (required)
  • Familiarity with Internet, email systems, and electronic health records (EHRs).
  • Professional demeanor and phone skills in working with persons served, team members, and community collaterals.
  • An active Driver\'s License and access to reliable transportation (required)
  • Willingness to transport clients as needed
  • English-Spanish speaking (preferred)
