Position Details

The Dual Diagnosis Treatment Team (DDTT) program offers support to children and adults who have both a serious mental illness and a developmental disability. Participants receive counseling, physical healthcare, support finding housing, assistance during crises, and help dealing with other issues in their daily lives.

The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with an Intellectual Developmental Disability and mental health diagnosis. The Case Manager will collaborate with other members of the team on the implementation of a recovery-oriented person-centered treatment and support plan as well as

performing initial intakes, assessments, and timeline summaries.

Providing intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, and natural support residences and also supporting data collection, monitoring interventions, and providing feedback to the treatment team regarding client progress.

The ideal candidate will possess the following qualifications:

Required:

• Bachelor's degree in social work, psychology, or another human service related field

• 1 year experience in a mental health setting and one year working in a developmental disability setting

Preferred:

• 3 years of experience in a mental health setting and one year working in a developmental disability setting
