General Summary / Overview Statement

Reporting directly to the BWH Director of Endoscopy Services, this position will provide medication management through an evaluation of therapy plans for patients to ensure safe, effective utilization that is tailored to specific medical conditions.

Work with patients, doctors, nurses and other health care professionals to avoid a variety of barriers associated with comprehensive drug therapy management including prior authorizations and ensure optimal patient outcomes. The individual will work closely with the BWH Infusion Service and its ordering providers to provide patient-centered care support for our population of approximately 15,000 infusion visits per year providing initial patient evaluation pertaining to prescribed therapies, and providing expertise in the practice of pharmacy.

  • Review infusion orders and therapy plans to ensure all drugs are appropriately indicated, effective, safe and convenient for the patient
  • Identify, resolve and prevent drug therapy problems that may lead to patient interactions or insurance denials
  • Works with the scheduling staff and patient financial services to develop workflows for triage and prior authorization review and directs work to be completed by the medication support coordinator(s)
  • Provides support to providers to complete peer to peer calls to reduce ordering provider involvement while reducing payer denials for care
  • Support the infusion service to educate patients and ensure that the patient understands the specific goals of therapy and that the patient and providers are working to achieve these goals
  • Support the infusion service to educate ordering providers on how to submit infusion orders and/or therapy plans appropriately and to limit payer denials
  • Work with Patient Financial Services to review high risk therapies (e.g., off label pharmaceutical use, changes in therapy plans, etc.) to prospectively prevent late cancellations due to failure to obtain prior authorizations in a timely manner
  • Perform initial patient evaluation, via electronic medical record and virtual means, to collect, analyze, interpret, and evaluate appropriate date relative to medication therapy prescribed
  • Ensure appropriate histories are obtained along with other patient data in the pharmaceutical care evaluation
  • Identify, resolve, and document patient specific drug therapy problems and issues in the patient's best interest
  • Evaluate the significance of signs and symptoms relative to disease processes and make recommendations for the modification of drug therapy
  • Evaluate the significance and clinical importance of adverse reactions and complications associated with drug therapy
  • Provide counseling and answer questions from patients, family, and health care professionals concerning drug therapy
  • Participate with physician, nursing and administrative leadership to evaluate and analyze patient outcomes from a pharmacy perspective
  • Maintain proficiency in the skills and theory of the practice of pharmacy by reviewing literature, attending in-services, seminars and conferences
  • Actively participate in departmental and infusion based clinical meetings as requested

  • Must have exceptional oral and interpersonal communication skills, to communicate effectively with members of other healthcare disciplines, and other Pharmacy staff.


  • Must be able to plan and coordinate the activities pharmacy staff in accordance with Policy and Procedures.


  • Must be able to properly motivate and supervise support personnel.


  • Priorities and organizes work while under stress and with time constraints. Must pay attention to detail virtually 100% of work time, and have critical thinking and decision-making skills.

  • Evaluates medical information and retrieves biomedical literature using appropriate search strategies and interprets biomedical literature with regard to study design, methodology, statistical analysis, significance of reported data, and conclusions.


  • Must have extensive knowledge and ability in all areas of the department including: IV Admixtures, Drug Information, Ambulatory, Manufacturing, and Drug Distribution.

  • Must be capable to assume the responsibility for coordinating pharmacy services on an assigned shift.


  • The individual in this position must demonstrate appropriate knowledge of principles of growth and development over the life span of the adolescent, adult, and geriatric patient. In addition, they must possess the ability to assess patient data relative to age specific needs and provide care as described in the department's policies/procedures.

    Qualifications
    Qualifications

  • Must have a Bachelor of Science degree in Pharmacy or Doctor of Pharmacy from an accredited college of pharmacy. Must be eligible for licensure to practice pharmacy in the Commonwealth of Massachusetts
  • Experience (2 years) managing patients' specialty medication needs
  • Clinical Pharmacist hospital practice experience (2 years) preferably in infusion services
  • Knowledge of payer policies preferred
  • Excellent verbal and written skills
  • Proficiency with Microsoft Office
  • Preferred License/Certification/Registration: CSP
  • Minimum Experience: Two years' experience as an institutional or specialty pharmacy (preferred) pharmacist
  • Additional Requirements: Excellent customer service and communication skills, proficient in the English language, ability to manage time and people effectively, good attention to detail, ability to follow policies and procedures. Commitment to applying individual clinical skills in collaborative team environment to achieve patient-specific and program-specific goals.
