Job Description

The Community Health Worker is a frontline public health worker that has a good understanding of the community in which our residents live. This position serves as a liaison between the residents and the, health providers and social service agencies in the community. The community health worker is responsible for facilitating residents' access to health services to improve their health outcomes. The community health worker also builds the residents health knowledge and self-sufficiency through a range of activities such as outreach, education, informal counseling, social support and advocacy. The community health worker will work closely with the on-site service coordinator to achieve the goal of having a healthy community.

Skills and Experience:

  • Knowledge of community health resources, services and health insurers
  • Knowledge of health related issues of older adults, such as physical and mental impairments common to the aging process
  • Experience working with older adults in a direct service capacity
  • Ability to establish trust, listen, assist residents with defining their health management issues, while helping them identify possible solutions
  • Demonstrate ability to advocate, organize, problem solve and provide results for residents served
  • Knowledge of Medicaid and Medicare programs
  • Advanced skills with Microsoft Office (Word, Excel, Outlook)
  • Experience working collaboratively
  • Excellent written, verbal and interpersonal skills

Major Functions:
  • Communicate with residents the purpose of their role and the impact the community health worker will have on their well-being
  • Assist residents with identifying their socio-economic issues that affect their overall health and develop health/social management plans and goals
  • Provide case management, link residents to needed health care information and services and provide follow-up care
  • Document all resident contacts and contacts made on their behalf in their electronic files, which will include resident progress notes, release of information documentation, assessments and other health related documents acquired on behalf of the residents
  • Translate and interpret health information for residents
  • Communicate residents' needs and issues with their health care providers
  • Provide health screening services such as monitoring blood pressure
  • Develop on-site health and wellness programs
  • Engage residents in on-site health and wellness activities
  • Support residents during hospital stays and/or doctor's appointments
  • Facilitate health education and behavior change strategies with individuals and groups
  • Communicate and work with community health organizations to assist with health navigation
  • Assist residents with completing health related forms and paperwork
  • Schedule health related appointments for residents
  • Provide advocacy to address health disparities that affect all residents
  • Assist with Medicaid and Medicare enrollment
  • Ability to travel is required
  • Ability to work flexible hours

Job Requirements

Qualifications:
  • Bachelor's degree with a concentration in nursing, public health, health and well-being, health sciences, or behavioral health preferred
  • Preference given to those who live in or near the community of the property's location and those who have completed a community health workers certification/credentialing program
