Purpose

The Cuban/Haitian Entrants Case Manager will provide case management services for Cuban/Haitian Entrants and other ORR eligible populations not benefiting from R&P services, under the Preferred Communities (PC) Refugee and Entrant program. This position recruits clients and assists them in accessing services and navigating local systems for 90 days, while also coordinating support activities in partnership with community sponsors. The position offers support and services to enrolled clients to help them reach self- sufficiency and successfully make a new home in the United States.

Responsibilities

  • Conducts community outreach to Cuban/Haitian Entrants and community members to identify referrals of Cuban/Haitian Entrant clients and need for support.
  • Enrolls into the program and manages a case load of clients, ensuring timely delivery of services and fulfillment of requirements outlined in the PC grant agreement.
  • Develops and implements intakes, assessments, and necessary referrals in consultation with clients and community sponsors, based on individual challenges/needs.
  • Performs periodic home visits and provides in-home orientation to clients, including in-home cultural orientation in cases when clients cannot attend class, due to distance or medical conditions.
  • Connects clients to in-house support programs and makes referrals to external service providers as necessary to assist clients in achieving self-sufficiency. Schedules appointments, including medical, school enrollment, and clothing bank, among others.
  • Provides interpretation and translation services to refugee clients, as well as transportation to appointments as necessary.
  • Documents all contacts and services in client case files and maintains case note logs. Ensures accuracy and compliance within case files of the Cooperative Agreements and the guidelines within the Office of Refugee Resettlement.
  • Tracks client progress and provides follow-up assistance throughout the service period. Prepares and submits required reports.
  • Assists the Community Sponsorship Supervisor in any duties he/she deems necessary.
  • Provides program updates and highlights to fellow team members during check-ins and staff meetings.
  • Effectively coordinates and works in tandem with other service providers to assist clients, avoid duplication of services, and maximize efficiency.
  • Effectively uses technology to maximize efficiency and quality of work during intake, needs assessments, and data entry.
  • Advocates for equal access to services for refugees and immigrants.
  • Keeps informed about refugee and immigrant issues and available services to clients, using current information to better identify and serve refugee and immigrant clients.
  • Attends weekly meetings with supervisor and other agency staff.
  • Works varied hours.
  • Undertakes other duties as assigned.

Qualifications

Education: Bachelor's degree is required.

Experience: Work experience and interest in refugee resettlement or other direct social services. Ability to provide culturally sensitive counseling and case management is required.

Special Requirements

  • This position requires in-person work at the CWS Durham office.
  • Fluency in English, Spanish, and French or Haitian Creole language another primary refugee language.
  • Computer literate, with proficiency in MS Office (Excel, Word and Access) and internet applications
  • Superior English written and verbal communication skills
  • Valid driver's license and insurance
  • Clean Driving Record
  • Access to Personal vehicle
  • Must be able to travel frequently and independently throughout relevant region.
  • Must be able to work evenings and weekends as assigned.
  • Must be able to pass a criminal background check and credit check.
