Position Summary:

The Director of Programs, in partnership with the Executive Director, provides oversight to Affiliated Partners of Community Care Partners (BH CP and LTSS CP). The role provides support and direction to BH and LTSS Care Teams in engagement strategies and in the provision of care planning and coordination for Mass Health Members with complex healthcare needs enrolled in an Accountable Care Organization (ACO) plan. The Director of Programs collaborates with Care Team Leaders and Directors of Care Coordination in an effort to promote integrated care, ensure quality and continuity of care, and support the values of person centered planning and Community First principles.

The Director of Programs is responsible for translating Mass Health and ACO/MCO requirements, and then developing and implementing policies, procedures and work flows that support Care Teams in meeting these requirements. With support from the Senior Project Manager and Program Manager, the Director of Programs develops of tools and resources needed to successfully meet Mass Health and ACO requirements and drives the training component across our Affiliated Partners. The role directs, oversees and supports an array of standing meetings: monthly meetings for Directors of Care Coordination including "CP Workgroup" and ACO Relations," project meetings as related to Health Equity Initiatives and Population health pilots launched internally or with ACO partners, meetings as related to partnerships that promote health equity such as with Medical Legal Partnership of Boston (MLPB), Learning Collaboratives for Care teams and regular meetings with external stakeholders such as CBHC partners.

The Director of Programs partners with the Executive Director in developing and managing ongoing relationship with MassHealth, supporting the Executive Director in meeting all MassHealth Deliverables on time. The role also collaborates with the Executive Director in developing and managing ongoing relationships with ACO partners consistent with the mission of MassHealth's Community Partner program.

Supervisory Responsibilities

  • The Director of Programs directly supervises the Senior Project Manager and Program Manager, and steps in for the Executive Director in his/her absence.
Job Duties and Responsibilities

The essential job duties/responsibilities of the position include but are not limited to the information listed below:

  • Partner with Executive Director in the oversight of our BH and LTSS CP Provider Partners.
  • Supervise Program Services Team, Including the Senior Project Manager and Program Manager.
  • Collaborate with Care Team Leaders, Directors of Care Coordination and the Medical Director to promote integrated care, and ensure quality and continuity of care provided by BH and LTSS care coordinators.
  • Translate Mass Health and ACO requirements, and develop and implement policies, procedures and work flows that support Care Teams in meeting requirements.
  • Oversee training development and implementation for all BH and LTSS Care Team roles that is delivered at central level, including New Employee Orientation, Annual Refreshers, Monthly trainings.
  • Serve as content expert in population health and health related social need content.
  • Develop tools and resources needed to successfully meet MassHealth and ACO requirements.
  • Collaborate with Executive Director in directing, overseeing and supporting regular external meetings with ACO and CBHC partners.
  • Oversee CP Team communications and resources provided in Program Services and Training sections of CCP's intranet, Jive.
  • Partner with Executive Director in developing and managing ongoing relationship with Mass Health and in meeting all Mass Health Deliverables on time.
  • Ensure timely information exchange, coordination, and integration of care with ACO and CBHCs and other external stakeholders.
  • Collaborate with the CCP Central team to analyze program data and implement quality improvements as indicated.
  • Promote the utilization of evidence-based and promising practices that optimize care integration and effective communication across systems of care, and that align with rehabilitation, recovery and wellness principles.
  • Ensure that the health-related social needs of each Enrollee are being identified and addressed utilizing a population health approach.
  • Perform other duties, as required.

Education and Required Credentials/Licenses:
  • Bachelor's Degree required. Master's Degree preferred.
  • Minimum of 5 years supervisory experience required.
  • Clinical and case management experienced required.
  • Effective skills in managing, teaching, and negotiating, and in collaborating with multidisciplinary teams.
  • A COVID-19 vaccination is a requirement of the position. One COVID-19 shot is acceptable, contingent on the individual receiving the second shot within the allotted time frame.
