Description:

*Evaluates and assess patients nutritional needs

*Formulates nutrition care plan according to nutrition assessment and standards of care.

*Asseses the effects of nutrition intervention

*Educates and councils patients requiring nutrition intervention

*Evaluates services and care provided to identify opportunities for improvement

*Communicates pertinent information to appropriate individuals

*Performs other job related tasks within job scope as requested by Supervisor

Minimum Qualifications:
* Bachelor's degree from an acredited college or university in foods and nutrition or dietetics.
* Completion of approved coordinated undergraduated program, dietetic internship, AP-4 program, or Master's degree with qualifying work experience.
* One year nutrition support experience required beyond entry-level training if primary responsibilities are in nutrition support.

Licensure:
* Registered dietitian or eligible for registration with the Commission on Dietetic
Registration. If registration-eligible, must take registration exam on first opportunity after hire date and pass exam within two consecutive opportunities to take the exam.
* L.D/N. (Licensed Dietitian/Nutritionist) or L.D/N. approved application from the
Department of Professional Regulation of Florida
