Description:

The Intake Coordinator for Mercy Home Health is responsible for communicating, coordinating and documenting care for the transition of patients to home-based services.

This role is also responsible for identifying / determining appropriateness of care, gathering medical, gathering /verifying insurance and financial data, documenting in the EMR and communicating with patients and families as well as working with Care Management, physicians and referral sources.

The Intake Coordinator is often the first point of contact for referral sources and people referred for Home Based Services. This role requires a high-level skill set for screening and referral triage, customer service, critical thinking and the ability to navigate multiple applications and computer systems.

Responsible for processing entry of all home care referrals, insurance information and service requests for Mercy Home and Community Health Services into agency Electronic Medical Record. Distributes referral information to each business entity daily.

Requirements include:

  • High School diploma or equivalent
  • Health care experience preferred
  • Excellent communication and customer service skills.
  • Displays optimal critical thinking skills.
  • Knowledge of Medicare rules and regulations preferred
  • Working knowledge of managed care environment preferred
  • Experience multi-tasking on multiple computer systems..
  • Must have valid Driver's license and reliable transportation to and from work site.
  • Opportunity to work from home. Must have reliable internet.**** It is work from home position, but could always change in the future.
  • Experience in working with Microsoft products such as Word, Excel, OneNote
