Overview

Job Description:

The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation, and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction, to promote continuity of care and cost effectiveness through the integrating and functions of case management, utilization review and discharge planning. The Inpatient Social Worker must be highly organized professional with great attention to detail, adaptable to frequent change, and compliant with regulatory and departmental guidelines and policies. Knowledge and experience with evidence-based practices, including trauma informed care framework. Knowledge and experience working with co-occurring populations (chronic health conditions, mental health and substance use).

Qualifications/Education/Licensure:

Master's Degree in Counseling, Master's degree in Social Work (MSW) or related field from accredited university. Resident of Counseling or Social Work within 6 months of licensure exam - Inpatient only. Strong assessment, critical thinking, and individual, family, and group therapy skills.

LPC, LCSW, or Ph.D with two years experience in mental health or substance use treatment.
