Interviews patients and triages phone calls in order to determine and assess patients' health care needs.
Develops plan of care or refers as necessary and provides direct nursing care to patients.
Documents nursing actions in patients' records in order to provide accurate medical records.
Reviews laboratory results and initiates appropriate actions and communicates results/messages to patients and others as appropriate.
Serves as an advocate and provides instruction, education, and counseling to patients.
Completes Medicare Wellness Visits.
Completes other duties assigned to him/her by the Nurse Manager.
Unit Specific Education/Experience/SkillsKnowledge equivalent to that which normally would be acquired by completing a four-year college degree program in Nursing; one to three years of related and progressively more responsible or expansive work experience in Nursing; or an equivalent combination of education and experience. The position also requires possession of a current license to practice as a Registered Nurse in the State of Michigan and BLS certification. May require a bachelor's degree in Nursing, work experience in a particular area of nursing or health care. This position requires or will require prior to the completion of the probationary period: knowledge of basic personal computer terminology and operations, and the ability to use e-mail and the Internet.
Desired QualificationsExperience in a Family Medicine outpatient clinical setting.

Summary of Physical DemandsThe job requires significant physical effort and involves fine manual dexterity, lifting and maneuvering patients, and extensive walking and standing 26-75% of the time; visual and aural acuity necessary to monitor patient care.
