The University of Miami/UHealth Department of SCCC has an exciting opportunity for a Full Time Nurse Navigator 1 to work at the UHealth Medical Campus.
The Nurse Navigator 1 acts as the initial point of contact for a patient entering the health system. The Nurse Navigator assures timely scheduling of the first appointment, coordination of care after completion of the first appointment, and will support the patient throughout the care continuum. The incumbent serves as a liaison between patients, families, caregivers, and the multidisciplinary care team. Furthermore, the Nurse Navigator demonstrates commitment to quality patient care, implements creative and innovative ways to meet the diverse needs of the patients and ensures best practices.
This is a Hybrid opportunity 3X a week onsite and 2X a week remote. Location: South Florida fire departments (Palm Beach, Broward, Miami-Dade, Monroe).
CORE JOB FUNCTIONS
Triages new patient appointments to the appropriate provider(s) and assures timely scheduling of initial appointments. Reviews outside medical records for appropriate scheduling. Assesses barriers to care and refers to support services, local, and national organizations when needed. Educates on the treatment plan for patients based on diagnosis. Supports patients throughout the care continuum. Counsel's individuals and patients on positive health practices. Collaborates with a multidisciplinary team of experts to outline the best treatment for patients. Performs holistic evaluation of specialty population, making use of enhanced proven techniques and procedures to achieve better results. Implements the improvement of patient care, and healthcare policies and resources. Mentors' other healthcare professionals by functioning as a preceptor or coordinating preceptors for visiting professionals, students, new graduates, and orienteers. Maintains professional knowledge by affiliating with professional and technical organizations, and participating in applicable continuing education programs, conferences, seminars, and workshops. Adheres to University and unit-level policies and procedures and safeguards University assets.
Department Specific Functions
Provide clinical care to firefighters in a mobile clinic setting onsite at various fire departments within the UHealth SCCC catchment area: Palm Beach, Broward, Miami-Dade, Monroe. Perform occupational medicine testing (vision screening, hearing screening, EKG, pulmonary function testing, etc.) Educate on treatment plans for common family medicine health conditions including but not limited to thyroid disorders, lipid disorders, blood glucose disorders, androgen deficiency, nutrient deficiencies, etc. Educate on cancer screening and FCI clinical recommendations according to increased cancer risk in firefighter population. Navigate patients to referrals, screenings, and other services as indicates. Provide navigation for newly diagnosed firefighters who contact the Firefighter Cancer Initiative. Collaborate with leadership on review and development of policies, procedures, and educational materials.

Education:
Bachelor's degree in relevant field
Certification and Licensing:
Valid State of Florida Registered Nurse (RN) license
Experience:
Minimum 2 years of relevant experience
