RESPONSIBILITIES:

Kforce has a client that is seeking a Patient Care Coordinator in Hollywood, FL. Responsibilities:

• Serve as 1st point of contact in establishing relationships with newly established patients
• Educate patients on company products and services to assist with conversions
• New Referrals (Fax and Phone) - Verify eligibility and authorization, CPT codes and place of service
• Re-authorizations - Send requests to PCP's and follow-up daily
• Map patient locations and assign them to the correct Field Clinician
• Work closely with Field Clinicians via phone/email to ensure assigned patients are informed and updated in a timely manner
• Arrange vacation coverage for patients
• Act as a liaison between the Field Clinician, PCP, Patient, and Health Plan rep and maintain thorough and accurate data entry notes on all communication
• Discharge Patients - In OS, send discharge letter to patient and inform billing department
• Comply with HIPAA regulations
• Communicates in a clear and understandable manner using appropriate verbal and non-verbal styles
• Attends required meetings and participates as a committee member as requested
• Assists with special projects as needed
• Maintains strict confidentiality

REQUIREMENTS:

• HS diploma/GED or equivalent; Associate's degree preferred, but experience is more important
• Proficient in Microsoft Office and Excel
• Excellent verbal and written communication skills
• Microsoft Suite abilities
• Computer savvy
