Description:

Trinity Health Of New England is looking for an experienced PCA/CC to join our team of mission-driven healthcare innovators. This person is responsible for assisting while providing exceptional treatment to patients with a variety of medical conditions.

The ideal candidate will display a drive to provide patient-centered experiences through scholarly scientific inquiry, compassion for the human spirit and mind, advocacy and education for the patient and families, and collaboration with all those who touch our patients.

Will assist with patient admissions including unit set-up and data collection. Perform activities related to patient's activities of daily living (i.e. patient's hygiene, ambulation and nutritional support) that incorporate age-specific, psychosocial and environmental needs. Responds to patient call lights promptly and courteously, transfer patients with the unit or to another unit. Transports patients for tests/procedures, discharges patients according to hospital guidelines. Relay patient clinical information to the patient's nurse promptly and refer patient/family questions and concerns to appropriate care team member.

Work Hours/Shift:

Full Time, 32 hours, day shift, must work every other weekend/holiday as scheduled.

You Will Be Responsible For:

Exceptional Patient Care: Contributes to overall quality of nursing care through performance improvement techniques that impact patient care positively.

Excellent Communicator: Communicate effectively (written and verbal) with admin, colleagues, patients and family member.

Strong Educator: Educate patients and family members so that they have a transparent understanding of the care being provided.

Technical Familiarity: Understanding in computer-based applications (E-MAR, clinical documentation), email, and standard applications such as Microsoft Office.

Active Teamwork: Demonstrates problem solving, conflict resolution, and positive communication through teamwork.

Requirements:

Education: High School Graduate or equivalent. Successful completion of Nursing Assistant

Certification: Must have current CT Nursing Assistant Certification

CPR Certification - BLS required

Ability to pass drug screenings and background checks. Read, write and speak English.

Preferred Skills:

Phlebotomy, operation of EKG equipment and some clerical skills would be a plus.
