The transplant clinical specialist ("the practitioner") provides consultative services to physicians, physician's assistants, nurses, care coordinators and other healthcare personnel in both inpatient and outpatient solid organ transplant patient care areas. The practitioner must be capable of:

• Assessing medical and drug literature;
• Evaluating transplant treatment protocols and participating in protocol development;
• Providing drug information to healthcare personnel in the area of transplantation and immunosuppression, with a focus on drug dosing, therapeutic drug monitoring, adverse events and drug-interactions;
• Providing patient education on transplant immunosuppressants and other medications used for the medical management of transplant recipients;
• Conducting scholarly activities, which include, but are not limited to, initiating and performing independent and/or collaborative research, publishing said research and/or review articles in the area of specialty and performing lectures locally, regionally, nationally and internationally based on clinical expertise;
• Acting as a liaison for the Department of Pharmacy Services to transplant Programs and the various medical divisions associated with organ transplantation;
• Precepting pharmacy practice residents and/or pharmacy students
  The practitioner will have responsibilities to both the transplant services (heart/lung/kidney/pancreas/liver transplant) and pharmacy. The practitioner will be working in the inpatient and outpatient settings with the transplant teams. The following is a brief description of the responsibilities of the practitioner in the various work environments:
  
• Attending team rounds;
  
• Providing drug information to the transplant team;
• Providing pharmacology consults for transplant patients in all therapeutic areas (i.e. immunosuppression, pain management, etc.); and
• Being available for patient education, which includes, but is not limited to:
  • Assessing for patient-specific barriers to adherence;
  • Developing an education plan (i.e. written vs. oral education, both methods, use of interpreter services) based on your patient's preferred method of learning;
  • Creating patient-specific educational tools; and
  • Providing counseling to patients and their family on their medical regimen prior to discharge.
• Attending transplant clinic as directed by the team;
• Providing drug information to the transplant team;
• Providing pharmacology consults for transplant patients in all therapeutic areas (i.e. immunosuppression, endocrine, general medicine, etc.);
• May serve as the dedicated practitioner to provide medication reconciliation within the epic for clinic patients;
• Being available for patient education, which includes, but is not limited to:
  • Assessing for patient adherence;
  • Providing counseling to patients on all new medications or changes to the current medical regimen.
• Pre-transplant Evaluation: The Transplant Pharmacist will complete a pharmacy assessment for each potential transplant recipient.The assessment may be done by review of the medical record. Live organ donors will receive pharmacy assessment as referred. Under the supervision of the transplant physician, the Transplant Coordinator ensures that the pharmacy assessment is completed, assisted by the administrative staff. The outcome of the assessment will be communicated to the Transplant multidisciplinary team and documented in the patient's medical record.

• Pharmacy assessment/intervention
• Medication education and reconciliation
• Medication Recommendations
• Selection/Care Planning: A Transplant Pharmacist will participate in multidisciplinary care and selection of transplant recipients/donors, including participation in Multidisciplinary Selection Committee meetings and Multidisciplinary Care and Discharge planning rounds.
  
  
• Waitlist Re-evaluation: A Transplant Pharmacist will re-assess transplant candidates following the Policy and Procedure for Maintaining the Waiting List and program specific Appendix, and/or as needed as identified during the pre-transplant, transplant/donation, or discharge phase.
• Admission for Transplant/Donation: The Transplant Pharmacist will assess transplant patients and donors to provide a patient specific care plan, as needed, during the transplant hospitalization and for discharge. The outcome of this assessment will be communicated to the Transplant multidisciplinary team and documented in the patient's medical record. The Transplant Pharmacist will attend the multidisciplinary rounds/discharge planning meeting.
• Additional duties May include Pharmacy:
  
• Medication usage evaluations (formal and informal);
• Writing and present formulary monographs for organ transplant medications (or medications within the practitioner's area of expertise);
• Implementation of pharmacy protocols that originate or cover your area of specialty (i.e. medication dosing/conversion protocols);
• Participation in pharmacy and/or pharmacy-related committees;
• Pharmacy staff education based on area of expertise;
• Pharmacoeconomic analyses of transplant medications and contract pricing (with or without contract negotiations).

Qualifications
PharmD with residency/fellowship training or a BS in pharmacy with equivalent years of experience
