At Help at Home, our QDDPs focus on coordinating the development and implementation of supported living services with referral sources, clients and employees, while ensuring the delivery of quality services. This coordination occurs with the direct supervision of employees, providing support for clients and ongoing communication to family, community and funding and licensure sources.

This role is located in Union, MO, but will require travel within the county.

A typical day for a Qualified Developmental Disability Professional includes:

Person-Centered Planning and Individual Support Planning:

  • Actively participating in the person-centered planning process, including development and implementation of Person-Centered Plans (PCPs) and the Individual Support Plan (ISPs).
  • Designing and implementing support and teaching strategies targeted at assisting clients with reaching their outcomes, as outlined in the client's ISP.
  • Providing supervision and training to Direct Support Professionals regarding the implementation of the PCP, ISP and teaching strategies.
  • Evaluating and revising support and teaching strategies, as necessary, to ensure progress toward achievement of outcomes and action steps.
Quality Assurance:
  • Reviewing all documentation, notifying the support coordinator of any incidents and providing oversight for DSP staff and team leads.
  • Maintaining communication with family members, guardians, staff, and support coordinators on a regular basis, particularly regarding policies, issues and concerns.
Supervision/Training:
  • Coordinating in-service training needed to assist staff in fulfilling responsibilities related to client-directed support.
  • Providing direct support by providing services needed to protect the health and safety of the client, especially in a staffing crisis.
  • Communicating with the Interdisciplinary Team about concerns and findings.
  • Being available to respond to staff in case of an emergency.
  • Performing on-site evaluations for all field staff.
Other job functions:
  • Client record-related maintenance and compliance.
  • Participation in On-Call rotation.
  • Coordination of client benefits including Medicaid, Medicare, SSI, SSA, and Food Stamps.
  • Securing community resources for clients.
  • Assisting the director with teaching the training classes and with on-site investigations, as requested, for worker injury and client feedback.
  • Maintaining and promoting a professional manner for self and support staff within all communications.
  • Assisting the director with other duties as assigned and with approval.

In order to be eligible for this role, you should have:
  • A bachelor's degree with one year of experience working with individuals with developmental disabilities, OR 5 years of experience working with individuals with developmental disabilities
  • Supervisory experience (preferred)
  • A personal vehicle that is insured and meets state safety/licensure requirements
  • A valid driver's license (may require chauffeur's license)
  • 24/7 availability for caseload-related crises
  • Availability to be On-Call
