Job Summary

  • The Value Based Arrangements Quality Coordinator shall work closely with and under the direction of the Manager, Government Payer Quality Programs or Designee.
  • This individual works outside of the office environment to improve quality performance in support of all THMG ambulatory Practices by identifying, monitoring, and closing care gap measures related to preventative and chronic care conditions, age appropriate screenings and immunizations.
  • This position supports THMG's commitment to reducing care fragmentation while optimizing patients' individual experience of care, improving health of populations, reducing costs of care for populations and enhancing care team satisfaction (Institute of Healthcare Improvement Quadruple Aim).
  • The incumbent will be required to build, interpret, and analyze reports as well as report out on recommend actions to be taken related to quality measures that intersect both Government and Commercial payers.
  • This position will serve as the primary liaison between the Government and Commercial payer programs.

  • 4 year Bachelor's Degree or combination of relevant education & experience may be considered in lieu of Bachelor's Degree
