We are seeking an innovative and self-directed registered nurse to join our newly designed Transitions in Care Team (TiCT). This team will enable Loyola to meet the current challenges of a quaternary referral center, including: workforce availability, patient complexity and increasing demand for services.

The TiCT is a team of registered professional nurses that will function at top-of- license to coordinate and navigate transitions in care and throughput efforts (admissions, transfers, and discharges) for a wide variety of multi-disciplinary care teams. The TiCT will be integral members of the team, and in collaboration with senior leaders have opportunities to contribute to and design new models of care. Join us and become #LoyolastRoNg!

The TiCT will function Monday Through Friday during day shift hours. There may be minimal weekend and/or holiday shifts, so flexibility is required.

What Registered Nurse RN will need:
  • A Degree from an accredited school/college of nursing is required - BSN or ADN
  • 3+ years acute care experience in general medical, surgical or intermediate care
  • Demonstrated expertise with a broad array of complex clinical experiences, which include facilitating complex transitions in care
  • BLS Provider (accredited by the American Heart Association) required prior to hire
  • Illinois Board of Nursing license prior to hire
