An exciting opportunity to become the RN Case Management Specialist at a growing Acute Care hospital in scenic Northern Arizona has just opened to external applicants.

The RN Case Management Specialist will feel extremely supported by this strong healthcare provider and the highly skilled caregivers they work with on a daily basis.  

Reporting to the Director of Case Management, the RN Case Management Specialist will join a dedicated team of Case Management / Utilization Review personnel who hold patient safety and care to the highest standard to ensure that all patients receive the care they need in order to heal properly.  

The RN Case Management Specialist will be responsible for maintaining swing bed patient records in compliance with regulations for documentation of care and services, follow all best practices in regards to admission, continued stay, and discharge requirements of each patient, help coordinate on-going care with community agencies and develop and update patient care plans as needed. The Specialist will assist with organizing across a multidisciplinary network in order to communicate clearly and to affect optimal patient outcomes, achieve continuity of care and reduce costs for readmission / unnecessary treatment. The RN Utilization Review Specialist is a vital connection between patients and the fantastic services this hospital provides.
