Responsibilities

Position Overview:

The Clinic RN, CDCES Care Coordinator is responsible for the coordination, integration, and facilitation of a robust diabetes disease management program. The RN, CDCES Care Coordinator will report to the Manager of Population Health. Key responsibilities for the RN, CDCES Care Coordinator include working with the Ambulatory Medicine senior leadership team to design, define, and implement strategic initiatives across the Ambulatory Medicine clinic system with regard to diabetes disease management and population health, leveraging analytics to help transform the health of our communities. This will include developing and monitoring disease management and care coordination processes and support primary clinical teams with these efforts. It will also include identifying the high-risk patient population and working more directly to ensure disease management and care coordination for this patient population. The position may involve some patient triage. The Clinic RN, CDCES Care Coordinator will work with the Manager of Population Health and Director of Population Health to develop this position to best serve the needs of the patient panel and the primary care teams.

Qualifications

Required:

  • Bachelor's degree (B. S.) from four-year college or university in Nursing or related science
  • Current California RN license
  • Current CDCES Certification
  • Current ACLS Certification
  • Current BLS Certification

Preferred:
  • Bilingual in Spanish
  • Familiarity and experience with information systems, analytical skills, clinical software including EHRs & Practice Management software
  • Previous experience in a large, complex medical ambulatory integrated system with multi-site environment
  • Two years Nursing experience
  • Knowledge of CPT coding, ICD coding, DNV, State Funded Programs, HMO and Medi-Cal
