The Abdominal Transplant Clinic at Massachusetts General Hospital is looking to hire an experienced nurse with interest in Kidney, Liver or Pancreas Transplant. The Post Transplant Nurse Coordinator reports to the medical and surgical directors of the abdominal programs with coordination and oversight of the Chief of Abdominal Transplant Surgery and the practice manager. The Coordinator is a registered nurse who is responsible for assuring competent, compassionate assessment and care of outpatients presenting to the Abdominal Transplant Clinic. The Coordinator serves as the central point of reference for all information about patients. Incumbent is responsible for:

  • Coordinating the administrative and clinical aspects of the patients, including patient care, education, quality assurance, research, administrative and financial activities, and consultation.
  • Screening patients, educating patients, coordinating the patient's plan of care, verifying test results and communicating between the patient and the physician.
  • Providing direct patient care, including administration of medications, IV therapy, infusions, injections, blood products and other clinical services.
  • Maintaining comprehensive documentation in Epic of transplant recipient progress through clinic, telephone and Patient Gateway encounters.
  • Assembling all necessary clinical, laboratory and other information and data to recommend a care plan and for review of transplant physician.
  • Communicating directly with all members of the multidisciplinary care team, including pharmacy, social work, nutrition, pharmacy, surgery, nephrology and hepatology.
  • Collaborating with inpatient nursing regarding patient admissions, discharges and follow-up after clinic hours.
  • Communicating with physicians on lab data and diagnostic results and implements changes in drug/treatment therapy as ordered by physicians and other health care providers. Clearly communicating with and educating patients on any changes to their care plan.
  • Participating in performance improvement, research, administrative and financial projects. This includes ongoing projects designing and implementing clinical protocols and procedures.
  • Continuing education in transplant-specific areas by attending internal, regional and national lectures and conferences. Maintaining an ongoing understanding of transplant-related literature and practice standards.

Qualifications
Qualifying candidates will have:
  • Current licensure as an RN in Massachusetts
  • Current BLS certificate
  • Bachelor's degree in nursing
  • CCTC greatly preferred
  • Bilingual - Spanish speaking preferred
