Description

The Supervisor, Care Management leads teams of nurses and behavior health professionals responsible for care management. The Supervisor, Care Management works within thorough, prescribed guidelines and procedures; uses independent judgment requiring analysis of variable factors to solve basic problems; collaborates with management and top professionals/specialists in selection of methods, techniques, and analytical approach.

Responsibilities

The Integrated RN Care Delivery Supervisor is responsible for leading an integrated ancillary care team that consists of RN Care Coaches, integrated behavioral health coordinators, and community social service support team members. The supervisor is responsibility for assisting in the development and execution of the integrated care team model. Responsible for ensuring that all ancillary resource programs are implemented, maintained, updated, based on population needs. This role is responsible for workforce management, onboarding new associates, training, and collaborating with various levels of management. The supervisor utilizes various strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.

Identifies and resolves barriers that hinder effective care. Ensures patient is progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations. May create member care plans. Decisions are typically are related to schedule, plans and daily operations. Performs escalated or more complex work of a similar nature, and supervises a group of typically support and technical associates; coordinates and provides day-to-day oversight to associates. Ensures consistency in execution across team. Holds team members accountable for following established policies and processes.

Required Qualifications

  • Registered Nurse (R.N.) with active nursing license
  • 3 or more years' experience in field case/care management
  • 5 or more years of previous management experience
  • Progressive business consulting and/or operational leadership experience
  • Previous experience working in a managed care field
  • Proven ability to work independently and demonstrate critical thinking skills
  • Exceptional communication and interpersonal skills
  • Able to demonstrate proficiency with Microsoft Office Products
  • Proficiency in analyzing and interpreting data trends
  • Flexible scheduling availability, with evenings and occasional weekends as business needs dictate
  • Must be passionate about contributing to an organization focused on continuously improving consumer experiences
  • This role is considered patient facing and is part of Humana's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.
  • This role is part of Humana's Driver safety program and therefore requires an individual to have a valid state driver's license and proof of personal vehicle liability insurance with at least 100,000/300,000/100,000 limits
Preferred Qualifications
  • Bachelor's degree in Nursing (B.S.N.)
  • Master's Degree appropriate to clinical specialty
  • Proven ability to lead teams and build strong working relationships, internally and externally
  • Experience leading remote or multi-location teams
  • Care Management Certification
