The Department of Social Worker at the University of Miami has an exciting opportunity for a Full-Time Social Worker to work at the UHealth Towers. The Social Worker functions as an Acute Care Social Worker-MSW for the Care Coordination Department under the direction of the Manager of Social Services and the Director of Care Coordination. The Acute Care Social Worker-MSW assesses, plans, and evaluates the transition of care needs of the patient related to discharge planning. The Social Worker will develop discharge plans according to the needs of the patients including but not limited to determining the eligibility for social programs and funding sources, assessing whether patients qualify for community assistance, and creating referrals for post hospitalization needs.

  • Addresses potential barriers to discharge.
  • Assess the patient's and family's psychosocial risk factors by determining prior level of functioning, support systems and ability to cope with illness.
  • Identifies patient's plan for transition to the next level of care.
  • Intervenes in cases involving child abuse/neglect, domestic violence, elderly abuse, institutional abuse, and sexual assault.
  • Assist patients with the completion of Health Care Surrogate and Living Will.
  • Assess the need for hospital-appointed proxy or permanent guardianship.
  • Accurately assess financial resources and deploys community resources as needed.
  • Identifies and refers patients with no other housing to the shelter.
  • Evaluate the need for medication assistance for unfunded patients.
  • Works with Health Fund to identify patients in need of Disability, Medicaid, ICP Medicaid, Medicare or HIX applications.
  • Conducts thorough assessments that address readmission risk factors.
  • Ensures services are confirmed prior to discharge to ensure readmission risk is minimized.
  • Arrange all hemodialysis and peritoneal dialysis.
  • Coordinate medications to be administered with dialysis.
  • Arrange transportation to and from clinics.
  • Assesses each patient for transportation needs during initial evaluation.
  • Identify transportation issues and proactively works with insurance and patient to resolve prior to discharge.
  • Address families that exhibit complex family dynamics that impact patient care and discharge.
  • Serves as a liaison between the discharge planner and physician to assist with post discharge form completion.
  • Provides a safe space and environment for all patients.
  • Uses reflective listening skills to engage and interact with patients and family.
  • Provides respect, empathy and compassion when needed.
  • Arranges family meetings on an as needed basis.

MINIMUM QUALIFICATIONS
  • Master's degree in Social Work
  • Minimum 1 year of experience
  • Certification in relevant specialty or field
  • Ability to communicate effectively in both oral and written form.
  • Ability to handle difficult and stressful situations with professional composure.
  • Ability to maintain effective interpersonal relationships.
  • Commitment to the University's core values.
  • Ability to work independently and/or in a collaborative environment.
