The University of Iowa Hospitals and Clinics is seeking to fill the Social Worker III Role within the Care Coordination Division, in the ICU. The ideal candidate will provide leadership and advanced social work practice skills in areas of social casework, program development, teaching and training, research and public education. They will collaborate with a variety of health care professionals to develop approaches to affect treatment options and assist in the coordination of social work activities related to facilitating movement of patients and families through the continuum of care. They will contribute to the social work profession, building on social work education and experience to enhance department initiatives and patient experience. This Team Lead role supports the functions of the work team by serving as a mentor and primary resource for social workers in the Care Coordination Division by facilitating the use of team building skills and collaboration among team members. This individual is responsible for daily operational practices, advocacy and education related to social work practice and professional development.

Duties to Include:
• Demonstrates initiative in problem solving and program development
• Provides informal and formal leadership
• Demonstrates knowledge of practice areas
• Demonstrates ability to develop the social work role
• Contributes to social work practice
• Identifies, conducts, and documents research and special projects
• Develops and implements plan of care, responsive to patient/family concerns, health care status, available resources, age-specific problems, and clinical impressions.
• Provides liaison and referral services to hospital resources and community agencies
• Develops and facilitates groups for patients and/or significant others as appropriate
• Develops working relationship with patient and/or family (e.g., engagement collaboration, advocacy, problem solving, etc.)
• Provides information, consultation, and professional support to interdisciplinary team members
• Documents according to departmental policies and procedures
• Complies with required standards for medical/legal reporting situations
• Performs other projects and tasks as assigned
• Participates in and utilizes supervision to assess skill level, identify areas for professional growth, and establish mutually agreed-upon goals
• Contributes to strengthening the social work profession
• Demonstrates initiative and provides leadership in developing services that respond to patient care demands and regulatory standards
• Provides informal and formal teaching to staff and other health professionals

Percent of Time: 100%
Schedule: Monday-Friday, 8-hour shifts between 8:00 AM to 4:30 PM, occasional evenings, weekend and holiday rotation.
