Under the direction of the Medical Director, the Nurse Coordinator and the Administrative Director of the Continuous Ambulatory Peritoneal Dialysis Unit (CAPD), the staff nurse cares for the CKD and ESRD patients. Responsibilities include clinical assessment, initiation of nursing plan of care and follow-up, clerical duties as they pertain to the Peritoneal Dialysis Outpatient Clinic, MGH in-patient clinical areas and the Center for Renal Education.

Principal Duties and Responsibilites:

  • Provides a professional environment in which to meet, interview and educate patients and their families about peritoneal dialysis, individualizing instructions as needed.
  • Develops individualized plan of care for each patient
  • Directs patients to education resources, including written, video and support group services
  • Assesses patient's emotional response and ability to cope with chronic kidney disease and the need for dialysis
  • Collaborates with social worker and renal staff
  • Educates and monitors patient for possible complications, i.e. exit site infection, peritonitis, catheter malfunction, fluid and electrolyte imbalance
  • Encourages patient and families' participation in plan of care
  • Changes transfer sets when appropriate, repairs catheter if necessary
  • Obtains cultures of peritoneal dialysis fluid and administers IV /IP antibiotics to treat peritonitis, exit site and catheter tunnel infections
  • Documents all patient activities, i.e., phone calls, teaching sessions and group meetings
  • Participates in the monthly QA/QAPI program based on data collected to address the issues specific to patients with CAPD
  • Participates in annual retraining and intermittent updates as new technology becomes available
  • Understands and practices the concepts of fiscal responsibility
  • Responds to patient care issues in an accountable and sensitive fashion
  • Attends staff meetings, weekly Dialysis rounds when appropriate, as well as MGH conferences related to the care of the patient with CKD
  • Seeks and maintains personal knowledge regarding kidney disease and associated problems in order to provide accurate resource information to patients
  • Triages telephone calls of patients, family members and MGH inpatient clinical staff.
  • Maintains and provides an on-call system for established MGH peritoneal dialysis patients
  • Calls insurance carriers when necessary
  • Frequent communication between patient, nurse, the patient's nephrologist and other care team members
Inpatient Nursing Responsibilities
  • Provides consultation services for the MGH Emergency Room
  • Provides support to inpatient peritoneal dialysis patients from both the MGH unit and outside units
  • Documents patient activities in medical record as necessary
  • Provides in-service educational sessions for the MGH nursing staff, Spaulding Rehabilitation Hospital, VNA, nursing homes, other rehabilitation environments
  • Reviews patient's medical and dialysis history with community health resources
  • Fosters communication between nurse, nephrologist and other health care team members
Center for Renal Education Nursing Responsibilities
  • Provides a professional environment to interview and educate patients with kidney disease
  • Interviews prospective candidates for the early intervention CKD program
  • Educates patients and families, individualizing instructions to their needs
  • Directs patients to written resources, appropriate support groups and educational websites and videos
  • Assesses patient's emotional response and ability to cope with chronic kidney disease
  • Collaborates with social worker and dietitian
  • Documents all patient activities, phone calls, teaching sessions and group meetings
  • Maintains knowledge base regarding CKD in order to provide accurate information to the patient.
  • Responds to phone call questions
  • Fosters communications between patient, nurse, physicians and other health team members


Qualifications
  • Minimum 3 years nursing experience
  • Current licensure as a registered nurse in Massachusetts required
  • BSN preferred
  • Nephrology nursing experience preferred
  • Ambulatory nursing experience preferred
Skills/Competencies:
  • High level of skill with patient care
  • Basic computer skills necessary
  • Excellent customer service and communication skills are necessary; ability to deal with diverse cultural backgrounds and to show flexibility and compassion when dealing with a variety of patients/families in stressful situations
  • Ability to work independently and as part of a team
  • Good organizational skills to handle multiple tasks in a busy environment including efficiency and throughput of patient volume.
  • Ability to effectively solve problems and identify emerging concerns and urgent matters and refer issues to management on a timely and appropriate basis
  • Assists in maintaining an environment of safe and comfortable patient care
  • Maintains CEUs and attends pertinent educational offerings to maintain high level of expertise
